 Lasik (Laser-Assisted Intrastromal Keratomileusis)
| The Lasik is a method that corrects mild and moderate myopia (nearsightedness), hyperopia (farsightedness) and astigmatism as an alternative to PRK, through the use of excimer lasers. Photoablation is effected within the internal corneal tissue after cutting and setting aside the outer protective layer which is later returned in place right after laser treatment. The laser reshapes the corneal surface creating a micro-lens in the pre-pupillary space. |

| The Lasik laser lifts a very thin sheet of corneal tissue through a special instrument (microkeratome) which is folded back on one side. Then the excimer laser treatment is effected as in a normal PRK. The part of the cut cornea is then zapped back on the treated surface where it spontaneously seals in a few minutes. |

The steps followed in the Lasik intervention procedures:
1. A few anesthetic drops are instilled.
2. The patient is made to lie on a reclining bed below the laser instrument.
3. A speculum is placed to keep the eyelid open.
4. A suction ring is applied to expose the cornea to the passage of the microkeratome to prepare the corneal flap.
5. The suction ring is removed.
6. The flap is lifted with a special spatula.
7. The optical center of the eye is “memorized” by an eye-tracker, while the patient focuses on a small point of red light.
8. The real and proper part of the treatment takes place:
- for myopia (nearsightedness) the laser flattens the corneal curve bringing back the image on the retina;
- for hyperopia (farsightedness) the laser will accentuate the corneal curve and reposition the focus on the retina;
- for astigmatism the laser reshapes the irregular corneal curve and refocuses all the multiple focal points on the retina.
9. The flap is zapped back in place, and the edges and margins of the flap are dried.
10. Medication with drops, and discharge without contact lenses or eye pads. |

Lasik can correct all degrees of (nearsightedness) myopia up to 8-10 diopters in relation to the corneal thickness, (farsightedness) hyperopia up to 5-6 degrees diopters in relation to the pre-surgery curve, and astigmatism even higher than 5 diopters.
The Lasik excimer laser treatment is not effected on the surface of the cornea but inside it. To be able to do this, before the excimer laser treatment is performed a thin incision of the layer is made with a mechanical blade, the microkeratome. This allows rapid and painless healing and very fast functional recovery. |

We may compare the Lasik operation to the opening of a book (the creation of the corneal flap by the microkeratome), then the extracting of some pages under the cover (carried out by the Lasik laser) and the closing of the book (putting back the flap layer).
There are no substantial differences in the type of anesthetic (always in drops) nor in the duration of the Lasik treatment with respect to the PRK treatment. In some stages of the Lasik operation, the patient’s vision may be temporarily blurred, a normal aspect that should not be worried about. At the end of the procedure, a therapy consisting of drops is prescribed which must be very diligently followed. |
